I agree that the information filled in this form can be processed and used in compliance with Romanian Law nr. 677/2001 regarding the protection of personal information during their processing and the free movement of this information. In addition, I understand I can use against the operator any of the rights I am entitled by the above mentioned Law nr. 677/2001, including but not limited to the rights mentioned at articles nr. 12 - 15 from this Law, namely the rights of access, intervention and oposition regarding processing of my personal information. Subsemnatul, conform datelor introduse mai sus, mă oblig ca în momentul schimbării unor date privind identitatea şi adresa mea sau proprietatea asupra câinelui/câinilor înscris/înscrişi, să aduc aceasta la cunoştinţa A.D.R.K. în maximum 7 zile de la data acestor schimbări.
Subsemnatul sunt de acord ca datele cuprinse în această cerere să fie prelucrate şi utilizate în conformitate cu Legea nr.677/2001 privind protecţia persoanelor cu privire la prelucrarea datelor cu caracter personal şi libera circulaţie a acestor date. Prin aceasta înţeleg să-mi pot exercita faţă de operator oricare şi toate drepturile ce-mi sunt conferite în acest sens de Legea nr.677/2001 menţionată mai sus, inclusiv dar fără a mă limita la drepturile menţionate de art.12 – 15 din această lege şi anume, dreptul de acces, de intervenţie şi de opoziţie în legătură cu prelucrarea datelor cu caracter personal. Scopul tuturor operaţiunilor de prelucrare a datelor de către operatorul A.D.R.K. este promovarea de servicii şi informaţii adresate direct membrilor A.D.R.K., prin mijloace de genul poştei sau mijloace de marketing la distanţă, altele decât modalităţile promoţionale obişnuite.
